Prior to your initial assessment with your Orthotist you will be asked to complete an intake form. We have provided you with the form here, if you would like to complete this prior to your arrival it will save you some time at your initial appointment.
Many of the orthoses that we fabricate at OrthoProActive are eligible for 75% funding through the Assistive Devices Program. The staff at OrthoProActive will help you to complete the ADP application at your appointment but the form is available for you here if you would like to have your physician sign this prior to your appointment. Keep in mind that ADP will only accept original documents; faxes and emails will not be accepted.
Wearing Schedule
It is important to ease into wearing your new orthosis. On occasion, the body will have difficulty adjusting to the corrective forces the brace applies to the body if applied all at once. That is why your Orthotist, at your fitting appointment, recommended a wearing schedule to allow for an acclimatization period to the new brace. A general wearing schedule has been attached here but always follow the instructions from your Orthotist.
